Extra-oral application of osseointegration

Abstract:
Objective. Since July 1988 a multi-disciplinary team in Birmingham has been created consisting of an ENT surgeon, a maxillofacial surgeon, maxillofacial technicians, audiologists, speech therapists and clinical genetics.Patients and methods. Patients requiring osseointegration areseen and assessed at special clinics and a treatment plan formu-lated. The Nobel Biocare system has been used exclusively.Results. Five hundred and sixty-eight patients have been referred of whom 374 had been implanted by January 1997. These include 255 bone anchored hearing aids, 150 auricular prostheses, 25 orbital prostheses, nine nasal prostheses and one scalp prosthesis. The overall fixture failure rate including the BAHA was 7.3%. The failure rate for the fixtures was 1.1% for auricular prostheses, 12.6% for orbital prostheses and 25% for nasal prostheses. The failure rate for the BAHA was 10.1% overall but the vast majority of these were in young and/or syndromic patients.Conclusion. This is an effective and successful method of treatment. Management protocols to reduce fixture losses and cope with the consequence of losses are evolving.Keywords: audiometry; otosclerosis; second ear; stapedectomy
